Six questions about patterns over the past six months. This is an informal screener covering the areas clinicians ask about — it cannot diagnose ADHD, but it can tell you whether a thorough evaluation is worth your time.

1. How often do you have trouble wrapping up the final details of a project once the interesting parts are done?
2. How often do you have difficulty getting organized when a task requires several steps?
3. How often do you forget appointments, deadlines, or obligations — even ones that matter to you?
4. How often do you delay starting tasks that require sustained mental effort?
5. How often do you fidget, squirm, or feel unable to stay seated when you're expected to?
6. How often do you feel driven by a motor — overly active, as if you can't wind down?

Assessment 2

Mental health check-in

Four quick questions based on a widely used, validated screening tool (the PHQ-4), covering anxiety and mood over the past two weeks. A screening check-in, not a diagnosis.

Over the last 2 weeks, how often have you been bothered by the following?

1. Feeling nervous, anxious, or on edge
2. Not being able to stop or control worrying
3. Feeling down, depressed, or hopeless
4. Little interest or pleasure in doing things
